Sarafem “Sam” Kakaes ARNP-BC
Founder | CEO
Sam Kakaes is a Board-Certified Advanced Registered Nurse Practitioner with over a decade of diverse experience, spanning primary care, medical procedures, emergency room settings, and the education of LPNs and RNs. As a published author, Sam has also developed courses aimed at enhancing the skills of medical professionals.
After years in the traditional insurance-based healthcare model, Sam became increasingly frustrated with its inherent limitations. The overwhelming focus on insurance billing, administrative burdens, and high patient volumes often hindered his ability to deliver the high-quality, personalized care he envisioned.
Motivated by a deep commitment to patient-centered care, Sam began exploring innovative healthcare models and found a perfect match in Direct Primary Care (DPC). He firmly believes that DPC is not just a model but a movement—one that fosters sustainable, long-term relationships between healthcare providers and their patients.
Sam’s vision is clear: to offer transparent pricing and direct access to comprehensive care, empowering patients to take control of their health. By prioritizing patient needs and minimizing barriers, he strives to revolutionize the healthcare industry, one patient at a time.
Kristine Snyder MSN, APRN, FNP-C
Nurse Practitioner
Kristine Snyder is a board-certified Family Nurse Practitioner who has dedicated her career to caring for patients in her community since 2010. After completing her Bachelor’s degree and working as a Registered Nurse at the bedside, Kristine obtained her Master’s in Nursing from the University of Tampa in 2019, specializing in family practice. As a Family Nurse Practitioner, Kristine can care for the entire family from infancy to the elderly. Kristine has a passion for women’s health and promoting a healthy lifestyle in both kids and adults. She holds a national board certification with the American Academy of Nurse Practitioners.
