Sarafem “Sam” Kakaes, is a Board Certified Advanced Registered Nurse Practitioner with over 14 years of experience. He has extensive experience in health and medical practices, ER, education, and vascular insertions. Most recently, Sam has become a published author and has written courses for medical professionals to learn how to open and run their own medical practices.

After working as a healthcare provider in a traditional fee-for-service model, Sam became increasingly frustrated with the limitations and challenges of the system. He found that the emphasis on insurance billing, administrative work, and patient volume made it difficult to provide the high-quality, personalized care that he was trained to deliver.

Driven by a desire to focus on patient’s needs, Sam began exploring alternative healthcare delivery models. As he learned more about DPC, he became convinced that it was the right fit for his patients. Sam sees DPC as a way to empower patients and create a sustainable, long-term relationship with them.

Nurse Practitioner

Kristine Snyder is a board certified Family Nurse Practitioner who has dedicated her career to caring for patients in her community since 2010. After completing her Bachelor’s degree and working as a Registered Nurse at the bedside, Kristine obtained her Master’s Degree in Nursing from the University of Tampa in 2019, specializing in family practice. As a Family Nurse Practitioner, Kristine is able to care for the entire family from infancy to elderly. Kristine has a passion for women’s health and promoting a healthy lifestyle in both kids and adults. She holds a national board certification with the American Academy of Nurse Practitioners.

Chandra Burkett

Office Manager

As an office manager, Chandra has dedicated her career to ensuring that patients receive the highest quality of care while managing the complex operations of a medical office. In her role, she oversees the daily operations of the medical practice, including managing staff, ensuring regulatory compliance, and maintaining efficient workflows.  Outside of work, she enjoys staying active and spending time with her family. “I am an avid reader and enjoy learning about new developments in healthcare management and administration.”

Hannah Perez

Asst. Office Manager

As an assistant office manager for a medical practice, her career has been shaped by her passion for organization and customer service. She began her career as a medical receptionist, working her way up through the ranks to become an integral part of the office management team. Hannah finds her job incredibly rewarding. “I have the opportunity to work with a talented and dedicated team of professionals who are committed to providing the best possible care to our patients. Every day, I am inspired by the compassion and commitment our medical professionals bring to their work.” In her free time, Hannah enjoys spending time with her fiancé and dog, going to the beach, and trying new experiences. “My experiences in the medical field have taught me the importance of balance and self-care, and I make it a priority to take care of myself both physically and mentally.”
